What is a Health Home?
At Project Hospitality, our Health Homes are groups of health care and service providers collaborating to ensure you receive comprehensive care to maintain your health. Your physical and behavioral health needs will be managed by a dedicated care coordinator. They will work with you, your support system, and all providers to ensure your medical, behavioral, and social service needs are understood and met.
Who are Care Coordinators?
Care Coordinators are trained to address all aspects of your well-being, providing support in areas you may not have received assistance before, including engagement in social and community activities. The assistance you receive is tailored to your goals and identified needs.

Who is eligible?
- Medicaid Recipients
- Must meet one of the following criteria:
- Two or more chronic health conditions such as asthma, diabetes, heart disease, cancer, or substance use disorder.
- Living with a Mental Health Condition.
- Living with HIV/AIDS.
Our Care Coordinators will:
- Organize and facilitate tailored services to address physical, mental health, substance dependency, and social service needs.
- Schedule medical appointments and review treatment plans to maintain good health.
- Support you in following your treatment team or provider’s recommendations.
- Identify community-based resources.
- Provide health tips to manage medical challenges.
Clients often receive medical services from multiple facilities. Your dedicated care coordinator aims to enhance your understanding of your conditions, improve your self-management skills, and emphasize the benefits of adhering to the treatment plan, ultimately promoting your overall health, wellness, and quality of life.
